What are the Pre-Licensure BSN health requirements?
Pre-Licensure BSN Health Requirements
- Tuberculosis
- Submit a 2-step TB skin test or a QuantiFERON TB blood test. The read date of the 2-TB skin test must be 7–21 days apart.
 - Every year after, you will submit either a one-step TB skin test or a QuantiFERON TB blood test.
 - If you test positive, you will provide a clear chest x-ray (lab report required) from within the past 5 years PLUS a TB Questionnaire (within the past 12 months).
 - If a chest x-ray was submitted initially, only the TB Questionnaire is required every 12 months. A new chest x-ray will be required every 5 years.
 
 - CPR Certification
- Submit your current American Heart Association BLS CPR card: Basic Life Support (BLS) for Healthcare Providers. Adult, Child and Infant CPR with AED.
 - Your submission must contain both the front and back of the card and the card MUST be signed. Electronic cards are also acceptable.
 
 - Influenza
- Submit proof of your flu shot vaccine by October 20, annually.
 - The earliest you can submit a flu shot to apply for the upcoming flu season is August 1st and the latest is October 20.
 - Please note - this requirement will not impact your compliance from 5/1 to 10/20 as the flu shot is being made available.
 
 - Health Evaluation Form
- Have your health provider complete a physical on the Health Evaluation Form, then upload it to Complio. The physical exam must have been completed within the past 12 months.
 - Included in the Health Evaluation Form is a health clearance for a respiratory fit test. This is not the actual fit test, but your healthcare provider saying that you are well enough to be respiratory fit tested.
 
 - TDAP
- Submit documentation of a Tdap within the past 10 years. The renewal date will be set for 10 years from the administered date of the Tdap.
 
 - MMR
- Submit: 2 doses of the MMR vaccine dated from any time OR a positive titer for measles, mumps, and rubella.
 - Note: If you submit a non-immune titer for measles, mumps, or rubella, you must submit 2 MMR vaccines dated after your titers.
 
 - Varicella
- Submit: 2 doses of the varicella vaccine OR a positive varicella titer.
 - Note: If you submit a non-immune titer for varicella, you must submit 2 varicella vaccines dated from after your titer.
 
 - Hepatitis B
- Submit: 3 doses of the Hep B vaccine or 2 doses of the HEPLISAV-B vaccine OR a positive Hep B titer.
 - Note: If you submit a non-immune titer, you must submit 3 hepatitis B vaccines dated from after your titer.
 - If you are receiving a 3-dose series, you will be compliant for 1 month following Dose 1, and for 5 months following Dose 2, to allow time for receiving the next dose. For a 2-dose series, you will be compliant for 1 month following dose 1.
 
 - CA CNA License
- Submit: A copy of your current CA CNA license OR a copy of the successful passing of both the theory and skills component of a CDPH-approved CNA program.
 
 - Covid-19 Vaccine
- Submit proof of your Covid-19 vaccine by October 20, annually.
 - The earliest you can submit a Covid-19 vaccine to apply for the upcoming flu season is August 1st and the latest is October 20.
 - Note: This requirement will not impact your compliance from 5/1 to 10/20 as the Covid-19 vaccine is being made available.
 
 - Drug Screen and Background Check
- Order a drug screen and a background check from American DataBank. Your results will be uploaded to Complio once completed.
